The role of AI Vigilance does not end with detecting Risks or Incidents. Results generated through Detect, Evaluate, and Analyze must be connected to the appropriate Response Path according to the nature of the Risk, its Urgency, scope of impact, and Institutional Responsibility. Particularly in a Black / Prohibited State, Execution Control and Institutional Response must remain clearly separated.

A Black State is a state in which the Governance Process has determined that an Action is not permitted. That Decision is connected to the Execution Boundary, where BLOCK is Enforced as the Default Response. However, the fact that an Action has been Blocked is not the same as determining that the Case is urgent. Black State indicates that Execution is not permitted, while Urgency Assessment determines how rapidly the Institution must respond.

A Case entering Black State therefore undergoes Urgency Screening. For an ordinary Prohibited Case, Execution can be Blocked, Evidence preserved, and the Case connected to established Reporting or Review Processes. Where a Case indicates an imminent and severe danger, such as Credible Mass-Casualty Planning, Imminent Suicide or Serious Self-Harm Risk, Actionable Kidnapping or Abduction Planning, Credible Terrorism-Related Threats, or Severe and Actionable Cybersecurity Threats, it proceeds to Immediate Escalation without waiting for the normal Reporting Cycle.

Urgency Assessment must function independently of Management or Commercial Interests. If responses to serious Risks can be delayed or suppressed because of Revenue, Reputation, Product Release Schedules, or Management Preference, Vigilance cannot function effectively as a Safety Function. High-Urgency Cases must therefore be capable of connecting directly to a predefined Institutional Governance Path, rather than depending solely on the normal Management Hierarchy.

Reporting Paths also differ according to the Role of the actor involved. For an AI Provider, the Case may connect to Processes relating to Safety, Incidents, or Model Behavior. Within an Organization, it may connect to Governance, Risk, or Oversight. In Regulated / Critical Sectors, additional Compliance or Sector-Specific Reporting may be required. Even for the same Incident, the required Output and Reporting Destination depend on who holds which Responsibility.

The core principle is:

Block ≠ Urgency Assessment ≠ Escalation ≠ Reporting.

In a Black State, Execution is first Blocked according to the already formed Decision. Urgency is then assessed independently, and the Case is connected to the necessary Institutional Response. This prevents the assumption that “the Safety Response is complete because Execution was stopped.”

AI Vigilance is not itself the mechanism that Blocks Execution. It also serves to observe and assess Operational Evidence, including Blocked events, and connect that Evidence to appropriate Institutional Action. The Execution Boundary Enforces the already formed Decision, while Vigilance feeds the Risk and Urgency of the event back to the Institution, connecting Technical Control and Institutional Governance within a unified Response Architecture.
